Deposit
Scheduling a surgical procedure at Dr. K Miami Plastic Surgery requires careful planning and coordination of various resources to ensure a seamless experience. By signing this Agreement, you acknowledge and agree that Dr. K Miami Plastic Surgery will incur certain costs and expenses to schedule and prepare for your procedure(s). Consequently, you are required to pay a one-time NONREFUNDABLE deposit of $1,000.00 (the “Deposit”) at the time of executing this Agreement. You agree and acknowledge that the Deposit SHALL IN NO EVENT BE REFUNDABLE, UNDER ANY CIRCUMSTANCES. This includes, but is not limited to, failure to obtain medical clearance.
The Deposit is valid for one (1) year from the execution date of this Agreement. If your procedure(s) are not completed within this time frame, additional charges may apply, and the prices quoted herein may be subject to change.
Payment and Terms
The total amount due (the “Amount Due”) covers only the following: pre- and post-operative visits and the procedure(s) listed above. All additional services, such as blood work, imaging, medical clearance, prescriptions, and other ancillary services, must be obtained at your own expense.
The full Amount Due must be paid in full 2 weeks prior to the scheduled date of your procedure(s). Failure to remit the full payment by the specified deadline will result in the cancellation of your procedure(s). You are responsible for all payments, including any rescheduling or cancellation fees. If someone else makes payments on your behalf, they must also sign this Agreement, thereby agreeing to be financially responsible for any payments made, subject to the cancellation policies outlined in Section 3.
Please note that medical results are not guaranteed. Your obligation to pay the Amount Due and fulfill any other obligations under this Agreement remains in effect regardless of the outcome of your procedure(s). Payment is for the services provided, not the results. If you are dissatisfied with the results, your treatment, or have any payment concerns, please contact Dr. K Miami Plastic Surgery at info@drkmiamiplasticsurgery.com. You agree to mediate any payment disputes before pursuing a chargeback from any third party.
For revision surgery scheduled within one (1) year from the original procedure date, you will be responsible for costs such as operating room fees, supplies, anesthesia, surgeon’s fees, and other associated expenses. If the revision surgery is scheduled more than one (1) year after the original procedure, full payment will be required, and any applicable price changes will apply.
Cancellation
In the event that you cancel your procedure(s) for any reason, the following policies will apply:
(a) 30 days’ notice.
If you cancel your procedure(s) with at least thirty (30) days’ notice before the scheduled date (or at any time if no date has been scheduled), you will be entitled to a refund of the Amount Due, minus the Deposit. If the Amount Due has not been paid in full, your refund will consist of the amount paid to date, minus the Deposit.
(b) 15-29 days’ notice.
If you cancel your procedure(s) with less than thirty (30) but at least fifteen (15) days’ notice, you will be entitled to a refund of fifty percent (50%) of the Amount Due, minus the Deposit. Your refund will be reduced by the Deposit and any costs incurred from third-party providers, such as implants.
(c) 0-14 days’ notice.
If you cancel your procedure(s) with less than fifteen (15) days’ notice, you WILL NOT BE ENTITLED TO ANY REFUND WHATSOEVER.
You may choose to reschedule your procedure(s), subject to a $1,000.00 rescheduling fee (the “Rescheduling Fee”) in addition to the Amount Due. Should you cancel a rescheduled procedure, a $1,000.00 cancellation fee will apply for each cancellation.
Refunds and Claims
To minimize disputes, claims, and chargebacks, Dr. K Miami Plastic Surgery has implemented a Zero Tolerance policy. If you wish to request a refund or have changed your mind about undergoing a procedure, please email our accounting department at info@drkmiamiplasticsurgery.com. Refunds and claims will be processed within 21 business days.
